Supporting families outcome framework
What is the supporting families programme
The Supporting Families Programme in Buckinghamshire is part of the early help offered to children, young people and families.
This programme works with the whole family, either through one professional working with each family member or through a Team Around the Family approach.
The programme requires:
- an assessment has been done that considers the needs and voice of the whole family
- an action plan that includes all relevant family members
- a lead professional for the family who is recognised by the family and any other professionals involved
What are the main outcomes
The national Supporting Families Outcomes framework has ten main outcomes.
- Getting a good education
- Good early years development
- Improved mental and physical health
- Better-managed substance use
- Improved family relationships
- Children safe from abuse and exploitation
- Families diverted from crime
- Safe from domestic abuse
- Secure housing
- Financial stability
The lead professional will ensure that these ten themes are explored with the family alongside any other concerns the family may have. The help the family needs to change these areas will be included in the family’s action plan.
